Whether you have been referred by an outside agency or are
coming here on your own, we understand you may be anxious when
contacting Quest for services. Our commitment is to treat you
with compassion and respect as we gather the information
necessary to provide you with the appropriate care. Before any
treatment can begin, you will first be scheduled for an
Assessment. Your counselor will then make recommendations
based on your assessment. If an assessment has already been
done by a referring agency, please continue to follow the
instructions below. Your counselor will review the referring
agency’s assessment and conduct a shorter interview with you
to make recommendations for your care.
Your Assessment When you first contact Quest to
schedule your assessment, a scheduler will ask for your name,
address, phone number and other personal information necessary
to register you into our system. Please inform us at this time
if you do NOT live in Stark County.
Once we have obtained your personal information, you will
be scheduled for an Assessment appointment at the Quest
location (Canton, Massillon, or Alliance) that is most
convenient for you. This appointment will take approximately 2
hours.
When you first arrive at Quest, you will meet with a
financial administrator to fill out the appropriate paperwork.
Reading and writing assistance is available. Please bring the
following paperwork with you:
- Proof of Social Security number (ID card or other
paperwork listing SS#)
- Proof of address (bill statement or driver’s license)
- Proof of family income (pay stubs or tax statements)
- Health Insurance cards or ID #s (this includes Medicaid,
Medicare and private insurance)
After the paperwork is complete, you will meet with a
licensed counselor for approximately 1—1.5 hours. The
counselor will talk with you regarding your physical history,
legal history, drug and alcohol background, and current use.
Based on this assessment, a recommendation will be made
regarding treatment. Quest offers adult and adolescent
outpatient treatment, intensive adult outpatient treatment,
and residential treatment.
Paying for Treatment Financial issues
are not a reason to delay or stop seeking
treatment.
Quest Recovery and Prevention Services provides services to
Stark County residents on a sliding fee scale based on the
individual’s ability to pay. Fees are determined by family
income and the number of dependents living in the home (based
on tax-filing status). Quest accepts most private health
insurance as well as Medicare and Medicaid. If you
are having financial difficulties, please talk with your
counselor. Flexible payment plans can be arranged.
